Reminder from Nurse Brenda for 2025 - 2026 School Year

Important Information from Nurse Brenda Bowers, RN, BSN

Dear Parents/Guardians,


The following forms need to be completed and submitted to Wawasee High School nurse
by the first day of school, August 14, 2025. State required Immunizations need to be up to
date prior to school starting. Any student not current with immunizations or who do not
have a current waiver on file will be excluded from school starting September 1, 2025.


Emergency Action Plans
Student’s requiring Emergency Action Plans include any student with the following:
1. Allergies requiring prescription Epi-Pen and/or Benadryl.
2. Asthma with prescription inhalers.
3. Seizures.
4. Diabetes.
5. Any other diagnosis requiring accommodation at school.


Please identify the appropriate Emergency Action Plan and additional forms, available on
the corporation website and have them completed by your primary care provider. Forms
must be submitted with both parent and physician signature to be valid. Students
receiving medication for their diagnosis will need to have a completed Medication Consent
form by a parent. This includes epi-pens and inhalers. If a student carries his own
emergency medication, rescue inhaler, or epi-pen, please complete the Permission to
Carry form with your physician.
This form authorizes your student to keep these emergency medications with
them while on school premises.


Please submit these forms to the school nurse:
Medication
Per Wawasee Community School Corporation policies and procedures, all prescription
medication must be kept in the health center. If your student plans to take medication
while at school, please complete the Medication Consent Form and submit medication in
original pharmacy bottle with label detailing student name, prescriber, medication, exact
dose, and schedule by the first day of school to the health center/nurse office. High school
students are allowed to transport medication to and from the school but must give
medication, upon arrival at school, to the school nurse. Students are not allowed to keep
medication in their lockers or carry medication while at school unless they have a
“Permission to Carry” form on file with the nurse.


Immunizations and Immunization Waivers:
Per Indiana State Law and the Department of Education, all students attending Wawasee
Community Schools are expected to be up to date with required vaccinations for school.
If you choose not to receive required vaccinations please complete the waiver form for the
new school year and submit it to the school nurse. A completed waiver form is required
each school year.


If immunizations are not up to date, or the annual vaccination waiver is
not on file with the school nurse by September 1, 2025 those students will
be not allowed to return to school.
